CMOH functions as a Patient-Centered Medical Home: a unique model of care that gives you personalized navigation and care coordination, enhanced access, and expert symptom management- starting with your first visit.
Our practice and our physicians are fully independent. This allows us to maintain relationships with most hospital systems and all academic institutions in Southeastern Pennsylvania.
Our patient navigators will act as your personal concierge, scheduling all of your external appointments for imaging, labs, specialist visits, etc. With this higher level of service, we can shorten your timeline of uncertainty, empowering your care team to receive prompt results and provide you with critical explanations, predictions, and plans of care. We also coordinate all 2nd opinions, matching surrounding institutional expertise and research studies with your specific needs.
Throughout the course of your care, our telephone triage team is available around the clock to assist you with at-home symptom management, and to schedule a same- or next-day appointment for you, if necessary.
And finally, our financial counseling team will assist you with any insurance concerns.
